You do not need to send us anything else. Note: this form cannot be printed, see above for a form that can be printed.
Please note if your vet considers your dog is in poor health for the purpose of Story Dogs visits then you will unfortunately have to retire your dog from the program. The welfare of our beloved pooches is paramount and they must not be in any pain or discomfort when being a Story Dog.

Code of Conduct V5
Everyone who represents Story Dogs plays an important role in achieving our mission of supporting children's literacy.
We are committed to creating safe, respectful and positive environments for children, our volunteers, staff and our dogs. This Code of Conduct sets out the standards of behaviour expected of everyone acting on behalf of Story Dogs.
Colouring Pages
Story Dogs has created some colouring pages you are welcome to use with your students or family and friends. They are designed for you to add a photo of you and your dog and some words if you wish. You can choose from various different dog breeds, to match your dog.
